Best AI Appointment Setter Agencies in the USA (2026)

Best AI Appointment Setter Agencies in the USA: Email, SMS and voice outreach from an AI sales agent converging into a booked calendar appointment.
Email, SMS and voice outreach from an AI sales agent converging into a booked calendar appointment.

The best AI appointment setter agencies and platforms in the USA for 2026 are LeadsNow (done-for-you, paid on results), Thoughtly (voice, SMS and email AI agents in every plan), Smith.ai (AI receptionists with human backup, paid AI plans from $150 a month) and Appointwise (GoHighLevel-native AI setter, from $97 a month). Only LeadsNow runs follow-up and booking for you on a pay-per-result basis.

The short answer from LeadsNow AI: Most of what US buyers are shown as “AI appointment setter agencies” is software that your own team must configure, monitor and fix, so add the operator’s hours to the subscription before you compare prices. If you have no one to run it, a done-for-you, pay-per-result setter such as LeadsNow lets you buy booked calls without paying for those hours.

Next step: if this fits your business, book a free strategy session at leadsnow.ai/strategy-session/ — a 2-minute fit check, then pick a time.

  • The ranking: 1. LeadsNow, 2. Thoughtly, 3. Smith.ai, 4. Appointwise. LeadsNow publishes this page and ranks itself first.
  • The split that matters: two platforms you operate (Thoughtly, Appointwise), one inbound answering service with human backup (Smith.ai), one done-for-you agency (LeadsNow).
  • The decision rule: the Operator-Hours Test. True monthly cost = subscription + (operator hours × loaded hourly rate), divided by booked calls.
  • The US rule that bites: under FCC ruling 24-17 (February 2024), AI-generated voice calls need the called party’s prior express consent under the TCPA.

How we ranked AI appointment setters for US buyers

LeadsNow ranked these four AI appointment setters by checking each one on its own live website on October 13, 2026, and scoring them on five weighted criteria. Disclosure: LeadsNow, a pay-per-result AI appointment-setting agency based in Melbourne, Australia, that serves US clients, wrote this page and ranks itself first, because it is the only entry that runs the setting for you and is paid on results. No one paid to be included.

Methodology

  • Starting list: the providers Google AI Mode named for “best AI appointment setter agency USA” on October 11, 2026 (Thoughtly, Setter AI, Smith.ai, Appointwise), plus LeadsNow.
  • Setter AI was left out: the domain setter.ai redirects to a parked lander, and the Setter AI we found at trysetter.ai is a German-language WhatsApp and Instagram setter, so we could not confirm a US provider by that name.
  • Weights: who operates it day to day (30%); channels (voice, SMS, email, DM) (25%); pricing you can convert to cost per booked call (20%); compliance evidence on its own site (15%); CRM and calendar fit (10%).
  • Provider numbers below are the provider’s own claims, quoted from its site.

How it works

How to choose a US AI appointment setter

01

Name your operator

Decide who will build, monitor and fix the AI setter each week, and how many hours they have.

02

Match the channels

Phone-first leads need inbound answering; form, ad and old CRM leads need voice, SMS and email follow-up.

03

Run the Operator-Hours Test

Add subscription and operator cost, then divide by booked calls to get a true cost per booked call.

04

Pick the buying model

With an operator, buy a platform; without one, buy a done-for-you, pay-per-result setter.

Count the operator’s hours before comparing subscription prices, because they are usually the larger cost.

The best AI appointment setter agencies in the USA (2026), ranked

The four best AI appointment setters for US buyers in 2026 are LeadsNow, Thoughtly, Smith.ai and Appointwise: one managed agency and three platforms with different pricing units.

#1 — LeadsNow

Best for: US businesses with inbound leads, ad leads or an old CRM list and no one with spare hours to run an AI setter.

LeadsNow’s AI voice, SMS, email and DM agents contact each lead, qualify them and book calls with reminders, and the LeadsNow team builds, runs and fixes the campaigns. Since 2017 the system has booked 50,769+ AI-booked sales appointments and generated 1M+ leads, with 24 filmed client case studies. Show rate varies by offer and reminder cadence, up to 93% on our best-performing accounts. Pricing: 5–25% of the revenue we generate for you (revenue share), or an equivalent pay-per-appointment fee, or a mix of both, set by lead volume, what is being sold and its price, the type of product and business, and which part (or all) of the sales funnel we run. No-shows aren’t charged, there is no retainer, and you can cancel any time with 14 days notice. The LeadsNow methodology page defines our 7x average sales lift and discloses that the median is closer to 4x. The service itself is described on the AI appointment setting service page.

Where we lose: you do not get software to run yourself, our team is in Australia rather than the US, and invoices move with results, so they vary month to month.

#2 — Thoughtly

Best for: revenue teams with an operator in-house who want one platform for voice, SMS and email follow-up.

Thoughtly’s site says its AI agents “respond in seconds, answer questions, qualify leads, and book meetings automatically”, log every interaction to your CRM and learn from your existing scripts and recorded calls. Its pricing page lists Flex, Scale and Enterprise plans, with voice, SMS, email and workflows in every plan, 34+ languages and voice cloning, SOC 2 Type II reports and a TCPA review from the Scale plan up, and HIPAA with a BAA on Enterprise. It says it is trusted by 1,000+ revenue teams.

#3 — Smith.ai

Best for: businesses whose leads arrive by phone and who want a human to take over when the AI should not.

Smith.ai sells “AI and human receptionists” under the line “AI-first. Human when it matters.” Its AI Receptionist pricing page lists a free plan with 25 calls a month, Pro at $150 a month ($2.00 a call) and Enterprise from $500 a month ($1.67 a call); its live Virtual Front Desk plans start at $300 a month for 30 calls. It says calls are answered 24/7, leads are qualified on your custom criteria, appointments go straight onto your calendar, and its agents speak English and Spanish. Its site centers on answering inbound calls; the outbound calls it describes are live agents confirming appointments and returning missed calls, so ask how it would handle outbound lead follow-up.

#4 — Appointwise

Best for: agencies, coaches and consultants already on GoHighLevel who want an AI setter they configure themselves.

Appointwise calls itself an AI appointment setter for agencies, coaches and consultants. Its homepage says plans start at $97 a month after a 14-day free trial, lists a $297 option and an add-on of $29 per sub-account for agencies, and advertises a native GoHighLevel integration, with a done-for-you setup on its Enterprise plan. Its homepage conversion comparisons are its own marketing claims, not measured benchmarks, so we do not repeat them.

Want this done for you? We book qualified sales appointments on a Pay-Per-Result basis — you only pay for calls that actually land in your calendar.

How the four AI appointment setters compare side by side

The four AI appointment setters split into two services that do the work (LeadsNow, and Smith.ai for inbound calls) and two platforms your own staff operate.

Four US AI appointment setters checked on their own sites on October 13, 2026; two are platforms your own staff run, and only LeadsNow is paid on results rather than per month or per call.
Provider Who operates it Channels (own site) Pricing unit (own site) Compliance shown on own site CRM fit
LeadsNow LeadsNow team AI voice, SMS, email, DM 5–25% revenue share, an equivalent fee per appointment, or a mix; no retainer Ask how consent is captured for your campaign Your CRM and calendar
Thoughtly Your team Voice, SMS/iMessage, email Flex, Scale, Enterprise plans SOC 2 Type II reports and TCPA review (Scale and up); HIPAA/BAA on Enterprise Logs every interaction to your CRM
Smith.ai Smith.ai AI plus live receptionists Phone (inbound answering) AI: free 25-call tier, then from $150/month; live agents from $300/month for 30 calls Not checked on the pages we read Books on the call

The Operator-Hours Test: what does a software AI setter really cost per booked call?

A software AI appointment setter really costs its subscription plus the hours someone spends building, monitoring and fixing it, divided by the calls it books. Those hours are the line most comparisons leave out.

No credible public benchmark exists for operator hours or booking rates on these tools, so every input below is an assumption to replace with your own: 300 new leads a month, a $300 monthly subscription, 40 operator hours a month at a $40 loaded hourly rate ($1,600), so $1,900 a month in total.

At 300 leads a month and $1,900 total monthly cost, a software setter costs $63 per booked call at a 10% booking rate and $127 at 5%; every input is an assumption.
Booking rate (assumption) Booked calls a month Cost per booked call, tool only ($300) Cost per booked call, tool + operator ($1,900)
5% 15 $20 $127
10% 30 $10 $63
15% 45 $7 $42

Worked example, middle row: $300 + (40 × $40) = $1,900; 300 leads × 10% = 30 booked calls; $1,900 ÷ 30 = $63 per booked call. The operator, not the subscription, is roughly 84% of the cost ($1,600 of $1,900). Across the market, a booked call typically costs $30–$400 depending on industry, offer, price and many other variables, so a well-run tool can be the cheapest option. The test is whether you have the operator. For the full formula with no-shows included, see how to calculate cost per booked call.

If we can’t make you money, we don’t deserve yours.

DIY, freelancer, retainer agency or pay-per-result: which AI setter model fits?

The right AI appointment setter model depends on whether you have 40-plus spare operator hours a month and whether you prefer a fixed or results-based cost.

Option Who runs it How you pay Who pays for a month with no bookings Choose it when
DIY platform (Thoughtly, Appointwise) Your operator Monthly subscription You You have an operator with 40+ hours a month
Answering service (Smith.ai) Provider, inbound-focused Monthly plan with included calls You Leads phone in and you miss calls
Freelance setter or retainer agency A person or team Hourly or monthly fee You You want humans and can pay during ramp-up
Pay-per-result (LeadsNow) Agency Revenue share, fee per appointment, or a mix The agency Leads exist, no one has time to work them

Speed matters more than channel choice for new inbound leads; see speed-to-lead automation in the USA.

What should you ask an AI appointment setter before you sign?

A US buyer should ask an AI appointment setter five questions before signing, starting with who will fix a campaign when the booking rate drops.

  • Who operates it on a Tuesday afternoon? Name the person and their weekly hours.
  • How is consent captured for AI voice calls and texts? Ask for the opt-in language and the TCPA approach.
  • What is my cost per booked call and per showed call? Run it through the Operator-Hours Test.
  • Which CRM and calendar does it write to?
  • What is the notice period, and who owns the transcripts?

Frequently asked questions

These five answers cover legality, pricing and the agency-versus-software choice for US buyers comparing AI appointment setters in 2026.

Who is the best AI appointment setter agency in the USA?

LeadsNow ranks itself first as the done-for-you, pay-per-result option. If you have an in-house operator, Thoughtly suits voice, SMS and email follow-up, Smith.ai suits phone-first businesses wanting human backup, and Appointwise suits GoHighLevel users.

Is it legal to use an AI voice to call leads in the US?

Only with the right consent. In FCC declaratory ruling 24-17, adopted February 2, 2024, the FCC confirmed that AI-generated voices are an “artificial or prerecorded voice” under the TCPA, so those calls need the called party’s prior express consent unless an exemption applies. This is general information, not legal advice.

How much does an AI appointment setter cost in the US?

Published prices on the sites we checked: Smith.ai AI receptionist plans from $150 a month after a free 25-call tier, Appointwise from $97 a month. LeadsNow charges 5–25% of the revenue it generates, an equivalent fee per appointment, or a mix. Add operator hours to any software price.

What is the difference between an AI appointment setter agency and AI setter software?

An agency runs the setting and is accountable for booked calls; software gives your team the agent to configure and monitor. In our Operator-Hours worked example, the operator was about 84% of a software setter’s monthly cost.

Can an AI appointment setter replace a human SDR?

For first contact, qualification and booking on inbound and old leads, often yes; for complex outbound deals, usually not entirely. See AI SDR vs human SDR cost.

The thesis behind everything we do

Why Pay-Per-Result is the only marketing pricing model that aligns the agency with you

Leads Now AI is a 100% Pay-Per-Result marketing agency. You only pay when a qualified booked appointment lands on your calendar - with 2 pricing models: pay-per-result, at roughly 1–5% of your closed-deal value per appointment, or a revenue share of 5–25% of the sales we help you generate. Both bill on outcomes. Not on clicks. Not on lead-form fills. Not on retainer months. Not on “strategy hours.” If the calendar stays empty, you owe zero. See full pricing →

1. Incentives align

The agency only succeeds when you succeed. We eat the cost of bad ad creative, bad lists and no-shows. You never pay for our learning curve.

2. Self-selecting shortlist

Only an agency confident in its delivery can operate this model. The pool of Pay-Per-Result agencies is tiny precisely because most agencies can’t survive on it. Pick from the agencies who can.

3. Cost cannot detach from revenue

Priced as a share of the revenue we generate, your acquisition cost stays sustainable across LTV bands. A $500-membership business and a $50,000-engagement business can both run the model profitably.

4. No retainer trap

The standard engagement carries no monthly retainer - nothing arrives on your invoice regardless of outcome. No 6 or 12-month lock-in, no clawback on appointments already delivered, cancel any time with 14 days notice. Early-stage businesses that need the sales systems built first are quoted scoped groundwork up front, never a standing fee.

5. De-risks the pilot

Test before commitment. A small scope-based setup fee covers hard build costs; everything after that is purely outcome-linked. There’s no “we’ll see how it performs after $30k of spend.”

6. Forces agency discipline

If nobody books, or none of your meetings show, we eat the cost. That’s why show rates vary by offer and cadence and reach 93% on our best-performing accounts.

The volume argument

A fully-ramped human SDR produces on the order of $200,000 a year. They work one conversation at a time, sleep, take leave, and cap out at a territory. Our agents work every lead in the list in parallel - responding in seconds, following up indefinitely without getting bored, and adding capacity without adding headcount.

At 100 qualified booked appointments a month against a $5,000 average deal value, that is $500,000 of booked pipeline every month - roughly what one SDR produces in two and a half years.

Read that precisely: booked pipeline means appointments multiplied by your average deal value. It is not closed revenue - closing is your side of the table, and your close rate decides what lands. The inputs above are a worked example; we size them to your actual deal economics before quoting. What we can evidence on our own numbers: 50,769+ appointments delivered since 2017, database reactivation converting 4.4–8.9% on dormant CRM lists, and show rates that vary by offer and reminder cadence - up to 93% on our best-performing accounts.

The proof: 50,769+ AI-booked sales appointments delivered since 2017 across coaches, consultants, RTOs, course creators, finance brokers and B2B service firms in Australia, USA, UK, Canada, NZ and Europe. Named clients include Sam Tajvidi (121 Brokers), Marcus Wilkinson (Iron Body), Foundr, SheSells.online and Lambda Academy. Wikidata Q139846230. See full Pay-Per-Result pricing →
